Pre-Appointment Evaluation

An important portion of your first visit will be devoted to a full therapeutic evaluation, so that your therapist can determine the best treatment plan for you.  To help facilitate the evaluation process, please download, print and complete the medical history form and the consent form. 

Completing this form in advance of your first treatment will help to expedite the process when you arrive. If for any reason you are unable to print and fill out these forms in advance, please come to the clinic 15 minutes early so that you’ll have time to complete them before your session.
